What should I bring to my appointment?
To make the most of your appointment at The Shoulder Center please bring:
- copies of any previous x-rays, MRI scans, or CT scans of your shoulders
(please bring both the actual films and the report)
- copies of the operative reports and/or clinic notes for any previous
shoulder surgeries.
- a list of your current medications, including dosages and frequency.

and Agreement for Treatment and
Financial
Responsibility/Assignment of Benefits Forms confirming your understanding
of our policies and procedures.
